APPLICATION PROCESS

· EOC Counselor meets with individuals to describe EOC services
· CONNTAC application is given to those interested in receiving EOC services
· Application must be completed, signed and returned to EOC counselor with supporting documents
· Appointments are scheduled for individual counseling sessions
